id="anx_V"> PRILOGA V EU izjava o skladnosti EU izjava o skladnosti iz člena 47 vsebuje vse naslednje podatke:

1. ime in vrsto sistema UI ter morebitne dodatne nedvoumne navedbe, ki omogočajo identifikacijo in sledljivost sistema UI;

2. ime in naslov ponudnika ali, kadar je ustrezno, njegovega pooblaščenega zastopnika;

3. izjavo, da je za izdajo EU izjave o skladnosti iz člena 47 odgovoren izključno ponudnik;

4. izjavo, da je sistem UI v skladu s to uredbo in po potrebi z drugimi določbami ustreznega prava Unije, ki določa izdajanje EU izjave o skladnosti iz člena 47;

5. kadar sistem UI vključuje obdelavo osebnih podatkov, izjavo, da je sistem UI skladen z uredbama (EU) 2016/679 in (EU) 2018/1725 ter Direktivo (EU) 2016/680;

6. sklice na morebitne uporabljene ustrezne harmonizirane standarde ali druge skupne specifikacije, v zvezi s katerimi je navedena skladnost;

7. kadar je ustrezno, ime in identifikacijsko številko priglašenega organa, opis postopka ugotavljanja skladnosti in identifikacijo izdanega certifikata;

8. kraj in datum izdaje izjave, ime in funkcijo podpisnika ter navedbo, za koga ali v imenu koga ta oseba podpisuje izjavo, in podpis.

Only where applicable. Point 7 requires you to state the name and identification number of the notified body, a description of the conformity assessment procedure performed, and identification of the certificate issued.

Is the declaration of conformity per AI system or per product line?

The declaration is per AI system. Point 1 requires the name, type and any additional unambiguous reference allowing identification and traceability of the specific AI system.

Which standards must I reference in the declaration of conformity?

Point 6 requires references to any relevant harmonised standards or other common specifications in relation to which conformity is declared. These are the same standards referenced in the technical documentation (Annex IV, point 7).

Where do I store the EU declaration of conformity?

Under Article 47(2), the declaration must be available for 10 years after market placement. A copy must also be included in the technical documentation (Annex IV, point 8) and submitted for EU database registration (Annex VIII).
